PROJECT NARRATIVE
The COVID-19 pandemic significantly impacted the United States’ health care system and disproportionately
affected vulnerable populations (i.e., elderly people of color and rural). Home healthcare agencies (HHAs) and
nursing homes (NHs) provide essential care for a large number of elderly patients post hospital discharge. This
study will examine the impact of COVID-19 pandemic on care transitions between hospitals, HHAs and NHs as
well as patient outcomes. The study also examines the effectiveness of infection prevention and control
practices in HHA and NHs in reducing the impact of COVID-19.
